There were:
- Surgery scars on John’s and Robert’s faces.
- Foreign objects in Robert’s eyes and possibly in John’s eyes.
- Sections of their hair were darkened possibly to form the V’s and to cover up deep scars.
Replacing the periods with commas and using the photo’s description as the headline or title might have been attempts to hide this image from image searches.
